How to register for SystmOnline

SystmOnline is a great online service which allows you to order repeat prescriptions and view your medical records. The service is open 24/7/365 and can be accessed from your home PC, Tablet or Mobile phone in 3 simple steps –

  1. Request to register for SystmOnline using this online form below or by asking at reception.
  2. Wait to receive your login details. You do not need to contact reception once you have completed the form. (Our team may contact you to verify your ID before you receive these)
  3. Log on using your username and temporary password to access your online account.

Request Access via our Website 

Our Online Services, available for patients aged 16 and over, allows you to order repeat prescriptions and view your medical records online. The service is open 24/7/365 and can be accessed from your home PC, Tablet or Mobile phone.

All you need is a Username and Password provided by the surgery. Please complete the online form below and upload photographic proof of identification such as a Passport or Driving Licence AND proof of address such as a utility bill.

Once you have your log-in details, simply log-on to SystemOnline via our website or by clicking here.  You will be asked to change your password to something more memorable once you have logged in.

You can then use our online service, making future repeat prescriptions or appointments much easier and more convenient.

Register for NHS app

The NHS app allows you to

  • View your GP electronic health record including what your doctor or nurse have written about you
  • Nominate your preferred pharmacy
  • Check your Organ Donation status
  • Set up a linked profile (proxy access)
  • Register with “Be Part of Research”
  • Choose if data from your health records is shared for research and planning purposes

Access to Medical Records

How do I get a copy of my health records?

The Data Protection Act gives you the right to ask for a copy of the information an organisation holds about you. This right is commonly known as a subject access request (SAR). Requests for medical (health) records
  • A request for information from health records has to be made with the organisation that holds your health records – the data controller. For example, your GP practice, optician or dentist.
  • For hospital health records, contact the records manager or patient services manager at the relevant hospital trust.

We require an application form to be completed before any information can be released.

Completed applications received may take up to 30 days to be processed and the information supplied to you.

GP Health Records for un-registered or deceased individuals

NHS England is the data controller of GP health records where an individual is currently not registered with a GP or is deceased. These records are held by Primary Care Support England (PCSE) on behalf of NHS England. You can find the application form and details of where to send your request here https://pcse.england.nhs.uk/organisations/public/

 

Third Parties

We use a processor, iGPR Technologies Limited (“iGPR”), to assist us with responding to report requests relating to your patient data, such as subject access requests that you submit to us (or
that someone acting on your behalf submits to us) and report requests that insurers submit to us under the Access to Medical Records Act 1988 in relation to a life insurance policy that you hold or that you are applying for.

iGPR manages the reporting process for us by reviewing and responding to requests in accordance with our instructions and all applicable laws, including UK data protection laws.

The instructions we issue to iGPR include general instructions on responding to requests and specific instructions on issues that will require further consultation with the GP responsible for your care.
