We welcome new patients to  register at Maple surgery.

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. Please use the link below to access this service.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Still want to Register via Paper Form?

To register you need to completed a GMS1 registration form and a patient questionnaire. These forms can be found on this page or can be collected from the surgery

If access to your online record is required we will need to see a copy of photo identification when you drop the completed forms into the surgery.

If you would like another person to be able to access your online record on your behalf please complete the proxy access form on this page.

We would recommend that you check your postcode through our postcode checker above to ensure your address is in our catchment area.

We will only ask for ID if you require Online Access, however we do encourage patients to download the NHS App.

Children wishing to register as patients with the surgery should also have a parent or legal guardian registered with us.
